Marsupialization is a process of creating a surgical window in the wall of the cyst, followed by evacuation of the cyst and maintaining the continuity between the cyst and oral cavity, maxillary sinus or nasal cavity, this process is also called decompression or partsch operation.Only material removed is the part of the cyst used to create the window, cyst cavity lining is left intact.this process decrease the intraoral pressure and results in reduction of the sized of cyst.this process is used as primary treatment and enucleation is done later.

Indications :

  1. Amount of tissue injury : if the tissue involved is excessive and there is important vital structure likely to get involved if enucleation is done ex- enucleation resulting in oronasal or oroantral fistula.
  2. Surgical access: If there is difficulty in access and there is chances of a portion of cyst being left behind which may result in recurrence then its better to go for Marsupialization.
  3. Assistance in eruption of teeth: If a teeth is needed is the dental arch and it is unerupted an associated with the cyst, marsupialization can result in the continued eruption of tooth.
  4. Extent of surgery: In a debilitated patient marsupialization is good option because it is less stressful and easy for the patient.
  5. Size of the cyst: If the cyst is very large and may result in the bone fracture after enucleation, then marsupialization is good option until the considerable bone have occurred.
